How to apply the bootstarp styles to the Forms?

Hi,

I am trying to apply the bootstrap styles to the Forms. I am find it hard to apply customized styles to individual form fields. Appreciate any help in this direction. Thank you
for example: from the attached files, I trying to make the source look like target.

You are really asking a lot in this post. Could you at least post the CSS and HTML that you've tried already? Give us some code that we can work with.
