Not able to add connection

We have used liferay social office as our intranet portal. But we are getting issues while adding connections. Even if we add a connection and the user accepts the connection, the user still gets an option to add the same user.
I checked the SocialRequests table. Here the entry is added with type=12, but when the user confirms the connection request entry goes in SocialRelation table with type=2 rather than type=12.
Tried debugging but got stuck at ContactsCenterRequestInterpreter. I am not sure whether this interpreter is being used. I cannot debug as the website is live and cannot make changes to it.
We are not getting this issue on any other machine with same setup.

We are seeing the same issue. The connection requests come in correctly, but upon accepting the request(s), the connection does not actually appear.

Did you end up finding a solution to the issue?
