dynamic data list form with mail field

i'm using Liferay 6.2
i use dynamic data list form to create a subscription form which contain some fields :

First question: one of them is E-mail so i want that

this field contains automatically the mail of the connected user and this field can't be modified by the use when he fill the form fields.

First question: an other field is level(level1,level2,level3...) which is a list so i want that this list will contain dynamicly these levels which are the names of folders

created in docuemnt and media portlet that' mean when the administrator create a new sub-folder in a folder called LEVELS , the name of this subfolder will be

added to the list of the form.

can any body help me please how can i do this!!
