Email template in Account created Notification nt working fine

Hii,
On my website I have created emial template for registered users.Once user completes registration he gets email set in "Account Created Notification"...It's working fine for all registrations.But once one problem happened.One user got following 2 email templates after registartion completion.:-

First Template :-


Dear Sachin B Mor,

Welcome! You recently created an account at http://www.csi-india.org. You are associated with [$CSI_CHAPTER$] chapter.
Your Payment receipt Number is [$CSI_RECEIPTNUM$]

Your password is kwbcfdufh.

Sincerely,
CSI-India
[e-mail]

Second Template :-


Dear Sachin B Mor,

Welcome! You recently created an account at http://www.csi-india.org. You are associated with Mumbai chapter.
Your Payment receipt Number is 98345

What will be the reason behind this..Does anyone knows?? Pls reply as early as possible.
